Dr. Toscano is committed to helping maintain your oral health. We consider each patient a member of our extended dental family and want to be sure each visit is comfortable and pleasant.
Every new patient begins with an in-depth medical and dental health history. This helps us accurately evaluate your current periodontal health. No treatment is rendered during your initial consultation.
This appointment is intended to:- Gather information
- Listen to your questions and concerns
- Recommend a strategy to improve your oral health
To minimize your wait time, we ask that you provide the following information at your first visit:
Medical History - download our patient registration and medical history forms by clicking here, complete them prior to your appointment and bring them with you.
Radiography- to accurately diagnose your healthy, we require current x-rays and/or CT scans. If your referring dentist has taken radiographs in the last 24 months, please forward them to us. If you don’t have recent images, Dr. Toscano will take the necessary radiography.
Medical Issues - please notify our office prior to your consultation if you have a medical condition or concern that would prohibit or modify the nature of the visit.
New Patient FormsDr. Toscano is available Monday - Friday. Please allow 45 - 60 minutes for your initial visit. We’ll do our best to schedule that visit as soon as possible. If you have severe pain or need emergency care we’ll do everything we can to see you as quickly as possible.
We request 24 hour notice if you need to cancel your appointment. This courtesy allows us to reschedule your time for another patient. We do charge a fee for appointments that are missed with no prior communication.
NOTE: all patients under 18 years of age must be accompanied by a parent or guardian.
